This has the potential to cause botulism. Cans were removed by person in charge. Observed raw chicken stored next to loose green peppers in the saute prep top. This has the potential for cross-contamination from chicken juices. Peppers were discarded by food staff. Observed raw eggs stored above ready-to-eat foods in the walk-in refrigerator. Eggs were moved to the bottom shelf by kitchen staff to prevent potential cross contamination. Observed fried rice cooling at room temperature in the food prep area, which is not a proper cooling method. Rice was temped at 115°F. Rice was out for an hour prior to inspection and was put in the walk-in refrigerator for proper cooling to reach the required 41°F or below. Observed multiple foods in the first saute bain marie at 44°F (ambient temp of bain marie also 44°F). All items in the bain marie were discarded by person in charge to prevent pathogen growth. Do not store time/temperature controlled for safety food in that bain marie until it can be repaired and the temperature can be verified to be 41°F or below as required. Hot line cold wells all items observed to be at 45°F as opposed to 41°F or below as required to prevent pathogen growth. Ensure to mix ice with water in cold wells and to keep items with a lid to better maintain temperatures. Food facility does not have available sanitizer test strips to determine appropriate chlorine concentration at the upstairs bar 3-compartment sink. Sample pouch of 15 test strips provided for chlorine. Obtain appropriate test kit within 10 days. In the clean dish dry rack area, observed multiple food storage bins with leftover food debris inside. Ensure clean dishes are fully free of food. Bins were removed to be cleaned and sanitized. Observed biofilm in the interior of the ice machine, a food contact surface. This could potentially contaminate the ice. Clean and sanitize immediately. Clean the following items: The floor in the walk-in freezer of food debris and plastic; Defrost the walk-in freezer and clean the buildup of ice off the floors, walls, ceiling, and shelves; The soda gun holder in the basement. Observed dried peppers and tortilla being stored trash bags. To protect from contamination utilize only food grade bags, containers for storing of food products. Trash bags may not be used. Observed a beef roast sitting in a stainless steel container on the floor below the dishwashing sink. Do not thaw at room temperature. Do not store food items on the floor or below a source of contamination. Product was relocated to the walk-in freezer at the time of inspection. Thaw product using refrigeration, under a cold flowing water or as part of the cooking process. Observed elevated temperature of TCS foods in the service line cold well. Items included: Cut tomatoes 44°F and Verde sauce 44°F. Have the refrigeration unit serviced and repaired. Maintain all TCS foods in cold storage at 41°F or below. As discussed with the manager, staff are to be taking food temperatures daily to ensure safe cooking, cooldown, hot and cold holding. Product was placed in the unit 1 hour previous to the inspection. An ice bath was added to reduce the product temperatures at the time of inspection. Improper sanitizer levels: The level of chlorine sanitizer in the front service line sani pail was over 200 ppm. Product diluted at the time of inspection. Maintain the active level of free chlorine in your sanitizer solutions at 50-100 ppm. The level of QAC sanitizer at the three compartment sink was 150 ppm. Increase to between 200-400 ppm. Both items were corrected on site. Monitor sanitizer levels daily. Observed loose caulk in various section of the hood ventilation system. Remove all loose caulk. Reseal using a high temperature caulk. Observed pooled water in the two ice buckets. Proper dry ice buckets or keep inverted to allow for proper draining. Note: Ice bucket hangers were broken off. Consider replacement of these buckets. The dishroom handwashing sink soap dispenser was not working. Repair or replace Correct within 3 days.

Initially, staff unable to determine sanitizing process at the F’real machine. The equipment manual was found and reviewed. Inform/remind staff of operating/cleaning processes for the machine. Clean the canopy of the cooking exhaust hood above the steamers. The temperature of the hot water in the employee toilet room was only 65 F. Repairs were made on-site and temperatures found to be >85 F. Maintain. In the kitchen/prep area, several water-stained ceiling tiles observed. Replace tiles within one week. Facility is scheduled for a full roof replacement over the summer of 2024. Monitor ceiling tiles and replace any that become water-stained. Foods were date marked or voluntarily discarded during the inspection. Re-train staff. This is a repeat violation. Observed raw wood used as a shelf on top of canned goods rack. Remove or resurface raw wood to be smooth, non-absorbent, and easily cleanable. Dirty knives observed with dried food debris and stored on an unclean tray at the cook line. Additionally, clean dishes were stored on dirty mats and racks with food debris. Unclean dishes were removed and taken to be washed, rinsed, and sanitized. Clean and sanitize the shelves and mats used for clean utensils. Utensils must be taken to the warewashing area to be washed, rinsed, and sanitized after use. Re-train staff. Facility does not have a routine cleaning schedule for non-food contact surfaces. These surfaces are not cleaned at a frequency to preclude accumulation of dirt and soil. Implement regular cleaning schedule IMMEDIATELY. The kitchen and front food preparation area must be deep cleaned. All equipment (shelves, interiors, and exteriors) must be cleaned. All floors including under equipment must be cleaned. All walls must be cleaned. Deep cleaning is required especially at and under the cookline where grease build-up was observed. Low temperature dish machine rinse aid observed stored in spray bottle labeled “sanitizer” and according to person in charge is used on tables. Low temperature dish machine rinse aid can ONLY be used for its intended purpose and as directed on the label. As there are no handles on cups this can potentially result in contamination. Cups were removed and need to be replaced with scoops with handles with the handles kept outside of the food. Observed sprouts being stored at room temperature across from the prep top. Sprouts are a time/temperature controlled for safety food and must be kept at the required temperature of 41°F or below to avoid potential pathogen growth. Observed towels lining prep table and cooking surfaces. Towels can be easily soiled and result in contamination and cannot be used to store equipment and food on top of. Towels were removed by person in charge. Person in charge was unable to locate probe thermometer for measuring proper temperatures of food. During inspection a probe thermometer was located and calibrated. Utilize accordingly. The chlorine concentration in the sanitizing solution of the 3-bay warewash sink was 250+ ppm, rather than 50-100 ppm as required. A recipe was created with person in charge for dishwashers to follow. Post recipe above sanitizing compartment for proper sanitization of dishware. Observed a buildup of biofilm on the interior of the ice machine along the ice tray. This has the potential to contaminate ice with pathogens. Do not use ice until interior of the machine is cleaned and sanitized. Observed multiple food storage containers stored without being properly air dried and having water pooled inside. Pooled water in stored dishware is prone to contamination and must be properly air dried. Observed multiple children’s toys in the dry food storage area. Clean the following non-food contact surfaces: – Racks above prep sink have a buildup of dust/debris – Hood filters above grill have a buildup of dripping grease – Walls, shelves, and ceiling in walk-in beer cooler have a buildup of mold. Observed a mouse trap on the same shelf as food in the dry goods storage area. Trap was moved by kitchen staff to prevent potential contamination of food or food contact surfaces. Observed old mouse droppings throughout the basement of the food facility. Ensure to remove any excess cardboard, bags, and paper products that could act as potential harborage for rodents. Clean the mouse droppings and wash and sanitize the areas to prevent the potential spread of any pathogens. Facility utilizes weekly pest control services. Observed multiple glue traps, rodent box traps, and poison to control any potential harborage. Provide pest control reports from the previous two months and continue to provide reports. No food storage or storage of food contact surfaces occurs in the basement. No signs of active pest activity were observed in basement or food prep/storage areas (chewed food packages, nests, etc.). Observed a decomposed mouse in the basement (No food storage occurs in area). Ensure to remove any dead pests and clean and sanitize the area.

Do not use refrigerator until temperature is 41°F or below. Side of grill and table below grill were observed with grease residue. Clean within 3 days.
